Applewood Athletic Club Swim Team

My child/children listed above hereby has/have my permission to participate in practices, meets, and other activities as a member of the Applewood Athletic Club (AAC) Swim Team. I understand that this may involve my child/children riding as passengers in private vehicles. Should the need arise, I authorize the coaching staff to seek and authorize medical treatment for my child/children. I understand that they will try to contact me before doing so if practical. I also understand that pictures of my child/children may be taken and displayed on either the website or the team bulletin board, and I authorize AAC and the coaching staff to do so.

The undersigned parent or legal guardian accepts the responsibility for leaving any facility the AAC swim team may utilize, in an orderly manner, and agrees to pay for any damages or accidents caused by or incurred by his/her child/children during the use of any facility. These facilities include AAC and any other swimming or recreational facility. This includes participating in practices, swim meets, and other activities as a member of the AAC swim team and releases AAC and its members and representatives from any claims or demands resulting from any injury which may be sustained by the participant, regardless of cause.

For children aged 12 and under, I further acknowledge that my family is participating in pool membership for the year in which I am registering my child(ren) to swim at AAC.

In consideration of being allowed to participate in any way in the AAC swim team program, including its related events and activities, I acknowledge, appreciate and agree that: 

1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular skills, equipment and personal discipline may reduce the risk, the risk of serious injury does exist; and,
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES OF OTHERS, and assume full responsibility for my participation; and,
3. I willingly agree to comply with the stated and customary terms and conditions for participants. If, however, I observe any unusual significant hazard during my presence or while participating, I will remove myself from participating and bring such attention to the team and league officials immediately; and,
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS THE AAC Swim Team, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and leasors of premises used for the activity ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the full extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT FREELY AND VOLUNTARY WITHOUT ANY INDUCEMENT. I certify that I, as parent/guardian with legal responsibility for this participant do consent and agree to his/her release as provided above.

The coaching staff aims to create a positive environment for all swimmers in practices, meets, and social programs. As a result, the coaches fully expect all swimmers to engage in respectful participation in all events. If a swimmer fails to comply with the behavioral expectations, the coaching staff will address the issues with the swimmer and the swimmer's parent(s). Actions to address the issue may include the removal from the team. Please refer to the AAC Swim Team Philosophy and Core Value Statement.
